Can Dupilumab effectively manage chronic prurigo nodularis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been struggling with prurigo nodularis (PN) for several years, and despite trying multiple topical treatments and antihistamines, the itching and nodules persist. My dermatologist mentioned that it is a chronic condition, but I am hoping to find a more effective long-term solution. Could you explain what the latest treatments are for prurigo nodularis? I have read about newer biologics such as Dupilumab. How do they compare to traditional treatments like corticosteroids?

Additionally, I have tried various creams and phototherapy with limited results. Are there any newer topical treatments or non-steroidal therapies that are effective? Another major concern is the scarring caused by the nodules. Even when the itching subsides, I am left with dark marks and thickened skin. Are there any medical or cosmetic procedures that can help reduce these scars?

Lastly, since this condition is often linked to underlying immune dysfunction, are there any lifestyle changes, dietary modifications, or stress management techniques that could help control flare-ups?

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

Prurigo nodularis (PN) can be challenging to manage, but there are newer treatments available, such as:

  1. Biologic therapies like Dupilumab have shown promise in reducing symptoms and inflammation, offering an alternative to traditional corticosteroids.
  2. Non-steroidal therapies such as topical calcineurin inhibitors (for example, Tacrolimus) and newer topical treatments like Crisaborole may also be effective.
  3. For scarring, medical procedures like laser therapy, microneedling, and chemical peels can help reduce dark marks and thickened skin.
  4. Lifestyle changes, including a balanced diet rich in anti-inflammatory foods, regular exercise, and stress management techniques like mindfulness and meditation, can help control flare-ups.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
