Patient's Query
Hello doctor,
My father is 67 years old and is experiencing a severe skin condition. He scratches his skin to the point of bleeding, and the situation worsens during the night. A dermatologist has diagnosed him with prurigo nodularis; however, none of the creams or medications have provided relief. He is in constant pain, struggles to sleep, and some of his wounds appear to be infected. Are there more effective treatment options available? What measures can we take at home to alleviate the bleeding and itching? Additionally, he has diabetes; could this condition be exacerbating his skin issues?
Kindly help.
Hello,
Welcome to icliniq.com.
I read your query and can understand your concern.
Diabetes can significantly delay the healing process, making it prolonged and challenging. Additionally, prurigo nodularis in individuals with diabetes contributes to dry and itchy skin, often associated with advanced stages of the condition where neuropathy is present.
Given that some wounds appear to be infected, it is crucial to focus on managing the itchiness.
Recommendations:
For itch relief, antihistamines and hydrocortisone (either oral or topical) can be administered for a duration of 10 to 15 days. To combat skin dryness, regular application of a moisturizer is advisable, as it can help alleviate both dryness and associated itching.
For wounds that show signs of infection, it is important to clean them thoroughly and apply betadine ointment.
In terms of diabetes management, maintaining well-controlled blood sugar levels can help mitigate complications such as neuropathy.
Potential causes include: Elevated blood sugar levels
Recommended investigations: Blood sugar testing, including fasting and postprandial levels
For follow-up: Please share images of the affected areas.
Preventive measures: Ensure the skin remains moisturized to prevent dryness.
I hope this information helps you.
Thank you.
