Hi doctor,

My wife age 40 years becomes incredibly irritable, emotional, and difficult to be around for about a week before her period. I have heard terms like premenstrual syndrome and premenstrual dysphoric disorder, but I am not sure what exactly she is experiencing. Her mood swings and outbursts are putting a strain on our relationship. I want to be supportive, but I often feel like I am walking on eggshells. How can I distinguish between normal PMS and more severe PMDD? What are some ways I can help my wife during this time without seeming insensitive? Are there any treatments or lifestyle changes that could alleviate her symptoms?

PMDD (premenstrual dysphoric disorder) is a severe form of PMS (premenstrual syndrome) with symptoms affecting the individual's daily routine and may sometimes require a psychiatrist review. Although it is majorly due to hormonal fluctuations that precede the menstrual period, it could also be worsened by conditions like stress, poor sleep, and lack of certain vitamins, amongst others. For now, you can encourage her to have healthy sleep, exercise at least three times a week, avoid both physical and psychological stress within this period, and take multivitamins rich in calcium, magnesium, and vitamin B6. Additionally, she should avoid alcohol and smoking. If this fails to resolve her symptoms, then she needs to be placed on combined oral contraceptive pills.
