Hello doctor,

I engaged in unprotected oral sex with a sex worker, and although there was no ejaculation (she had complained of some throat issues), I have been experiencing episodes of nausea and vomiting that are intermittent. I also noticed blood in my sputum along with some flu-like symptoms. While the bleeding has ceased, and the flu is not recurring as frequently, I still occasionally experience flu-like symptoms. However, the persistent nausea is concerning to me. Should I seek medical evaluation and advice regarding these symptoms?

Unprotected oral sex does carry a relatively low risk of HIV (human immunodeficiency virus) acquisition. I recommend that you undergo HIV testing immediately to establish your baseline HIV status. Following this, it is advisable to get tested again at four weeks after your last unprotected exposure. If the result is negative, it is recommended to follow up with confirmatory HIV antibody tests at three and six months after the last exposure.

Additionally, I suggest considering a chest X-ray and a sputum AFB (acid-fast bacilli) smear and culture sensitivity test. It is also important to monitor your body temperature over two to three days.
