Hello doctor,

I have been experiencing continuous vaginal discharge for nearly three months. Initially, there was a strong, iron-like odor, which later changed to a slightly fishy smell. Before my periods, the discharge became thicker and whitish. Over the past few months, I have also noticed an increase in facial breakouts and significant hair loss. Please help.

What is bacterial vaginosis?

BV occurs when there is an imbalance in the natural bacteria present in the vagina. Typically, "good" bacteria, known as lactobacilli, help maintain a healthy environment. However, when there is an overgrowth of other bacteria, it can lead to BV.

Common symptoms:

  1. Unusual vaginal discharge.

  2. A strong, fishy odor.

  3. Vaginal discomfort or irritation.

    It is important to note that some women may not experience any symptoms at all.

    Diagnosis:

    To confirm the presence of Bacterial vaginosis (natural balance of bacteria in the vagina is disrupted, causing overgrowth of anaerobic bacteria and a reduction good bacteria, we recommend a high vaginal swab test. This will help us identify the specific bacteria causing the imbalance.

    The possible treatment options are:

    Effective treatment typically involves antibiotics. Based on your situation, the following regimen is recommended:

    1. Metronidazole: 500 mg taken orally twice daily for seven days.

    2. Clindamycin cream: two percent intravaginal cream applied once at bedtime for seven days.

      Please ensure you complete the full course of medication, even if symptoms improve before completion. Kindly consult your doctor and take medicines accordingly.

      Additional recommendations:

      1. Avoid using scented products in the vaginal area, as they can disrupt the natural balance of bacteria.

      2. Refrain from sexual activity during treatment or use protection to prevent potential recurrence.

      3. Maintain good hygiene and consider wearing breathable cotton underwear.
