Can biologic medications help manage ankylosing spondylitis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been living with ankylosing spondylitis for a few years now, and my recent blood work indicated elevated inflammatory markers, which worries me about disease progression.

My doctor has suggested that we consider starting a biologic medication to help manage my symptoms. However, I am concerned about potential side effects and how they might interact with my current pain management regimen, which includes NSAIDs.

Additionally, I have been experiencing increased fatigue and some difficulty with my morning stiffness, and I want to ensure that any new treatment does not exacerbate these issues. What should I be aware of when considering this new medication?

Kindly help.

Hello,

Welcome to icliniq.com

I understand your concern.

Ankylosing spondylitis is an autoimmune condition linked to HLA-B27 and typically has a progressive course. If left untreated, it can lead to severe joint stiffness, persistent pain, and, later on, difficulty in breathing and a general loss of independence.

You should discuss the biologics currently available with your doctor, as they are more effective in improving long-term outcomes compared to using anti-inflammatory medications alone. Your morning symptoms are most likely going to improve with biologics. The symptoms are unlikely to worsen, so selecting the right biologics is essential.

Please check with your rheumatologist about biologic drugs like Etanercept, Golimumab, and so on. These medications are more effective in controlling symptoms and improving outcomes. Although they are not the cure

To stay productive, it is important to engage in regular range-of-motion exercises, gentle stretches, and deep breathing exercises. Yoga can also help maintain flexibility.

Remember that treatment outcomes are not guaranteed, but with biologics and regular exercise, you can remain active and independent.

I hope you are satisfied with my answer. For further queries, you can consult me at icliniq.

Thank you.
