Hello doctor,
I am 20 years old. I have an uneven skin tone with blackheads and whiteheads. I have a dark neck, dark circles, dark spots on my face, and sometimes it is itchy on my nose. My period cycle is normal. I want a clear and even skin tone.

I do understand your concern.
It appears to be acanthosis nigricans. Apply Tretinoin 0.025 percent cream at night (on the neck) for two months. Use Salicylic acid-based scrub twice weekly for four weeks (on the neck). Please answer the following:
Have you recently gained weight?
Do you have any stress?
Was there any prolonged exposure to the sun?

No, stable weight is maintained, but after two years, I gained weight, and now I have lost. Yes, I am a little bit stressed and worried about family problems. I have to go to college, so I have to face the sunlight. I used sunscreen for about a month.

I suggest you the following:
1. Avoid stress. Relax and make yourself stress-free.
2. Velvety brown skin in the neck occurs not due to melanin but due to hyperkeratosis and abnormal thickening of the outermost layer of skin. It may be related to obesity, hereditary causes, diabetes, or drug-induced, such as oral contraceptives, steroids, nicotinic acid, or internal malignancy. It starts as pigmentation, dryness or roughness of skin, and velvety texture.
3. Practice regular exercise. Consume a low-calorie diet and reduce weight if you are overweight.
4. Affected area should be cleaned regularly with cotton balls.
5. Apply Tretinoin 0.025 percent cream at night for two months.
6. Wash face with Ahaglow face wash (purified water, acrylate copolymer, sodium laureth sulfate, aloe vera) twice daily for ten days.
7. Apply Cetaphil skin lightening cream (aqua, glycerin, niacinamide, butylene glycol, hydrogenated polydecene) twice daily for two weeks.
8. Take antioxidant tablets once daily after dinner for 30 days. Eat a diet rich in antioxidants, zinc, and vitamins A or C.
9. Drink plenty of water. Stay hydrated.
10. Avoid prolonged exposure to sun. Use Photostable sunscreen 30 minutes before going outdoors.
11. Avoid dusty and polluted environments.
12. Exercise regularly. Avoid oily, junk, and fast food. Avoid food that contains lots of sugar and carbohydrates.
13. Eat more greens and fresh fruits.
