Hi doctor,

I am a 32-year-old biological female. At the age of 20, I began testosterone therapy as part of a gender transition process to become male and to suppress estrogen. I remained on hormone replacement therapy (HRT) for ten years. Since discontinuing testosterone two years ago, I have been experiencing several distressing symptoms, including:

  1. Complete loss of libido.

  2. Vaginal dryness.

  3. Severe sleep disturbances (frequently waking between 2 to 3 a.m.).

  4. Occasional night sweats occur once or twice a month.

  5. Persistently dry eyes.

  6. Menstrual periods that last only one day.

I consider myself otherwise healthy. I work out daily, maintain excellent sleep hygiene, and follow a diet rich in fruits and vegetables. Given that these symptoms were not present while I was on testosterone, I suspected a hormonal imbalance as the underlying cause. I have had my hormone levels tested multiple times. Although they consistently fall on the lower end of the normal range, my doctor believes they are not responsible for my symptoms.
