What causes vaginal and perineal dryness?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have my perineum skin dry, and it keeps slicing like paper cuts. The paper cuts are present on the inner labia and all over the vagina. There are rips and bleeds during intercourse. I also have some burns. I am taking a contraceptive pill.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

It sounds like you are experiencing significant vaginal and perineal dryness, leading to painful microtears, burning, and discomfort. A few possible causes are

  1. Hormonal changes – The contraceptive pill can sometimes lower estrogen levels, leading to vaginal dryness and thinning of the skin.
  2. Lichen sclerosus or other skin conditions – Chronic thinning and tearing of the vulvar skin may be linked to skin disorders.
  3. Yeast or bacterial infections – Recurrent irritation, burning, and small tears can sometimes be signs of infections.
  4. Allergic reactions or irritants – Soaps, detergents, scented products, or lubricants may cause skin sensitivity.
  5. Autoimmune conditions – Conditions like Sjögren’s syndrome can cause excessive dryness.
  6. Sexual friction and lack of lubrication – If lubrication is inadequate, tearing can occur.

Recommendations:

  1. See a gynecologist or dermatologist for an exam to check for infections, skin conditions, or hormone-related changes.
  2. Use an estrogen-based vaginal cream if your doctor confirms hormone-related atrophy.
  3. Switch lubricants. Opt for a pH-balanced, water-based, or silicone-based lubricant.
  4. Apply a gentle, fragrance-free moisturizer (like coconut oil or a medical-grade vaginal moisturizer) daily.
  5. Avoid harsh soaps, scented products, and douching.
  6. Consider switching contraceptives. Some pills lower estrogen levels, causing worsening dryness.

Since you are experiencing bleeding and persistent symptoms, it is essential to consult a healthcare provider to rule out underlying conditions. Would you like help finding a doctor near you?

I hope this answers the query.

Please let me know if I can assist you further.

Thank you.
