Patient's Query
Hello doctor,
My uncle, who works on a farm, was diagnosed with scrub typhus after developing a high fever, body aches, and a rash. He started taking Doxycycline five days ago, and while his fever went down initially, it has come back and is fluctuating between 100 to 102°F. He also feels weak and complains of dizziness. His doctor said to continue the antibiotics for a full course, but we are worried because his symptoms are not completely resolving. Could this mean that the bacteria are resistant to Doxycycline? Should we consider a different antibiotic like Azithromycin? Also, his liver function tests were abnormal before starting treatment. Could scrub typhus be causing complications, or should we get additional tests to rule out another underlying condition? How long does it typically take for symptoms to fully resolve? Please give your suggestions.
Hello,
Welcome to icliniq.com.
I understand your concern.
Treatment and drug resistance of scrub typhus: Doxycycline is the primary treatment for scrub typhus. However, antibiotic resistance can occur. If your uncle's symptoms persist or worsen despite treatment, resistance is a possibility.
Alternative antibiotics: Azithromycin is an alternative antibiotic that may be effective against scrub typhus.
Liver function tests (LFTs): Abnormal LFTs can occur in scrub typhus due to the infection itself or the antibiotic treatment. However, it is essential to rule out other underlying conditions that may be contributing to the abnormal LFTs.
Complications and additional tests include: Scrub typhus can cause complications, such as:
1. Hepatitis.
2. Renal failure.
3. Respiratory distress.
4. Neurological symptoms.
Additional tests may be necessary to rule out other underlying conditions, such as:
1. Liver ultrasound.
2. Viral hepatitis tests.
3. Autoimmune disorder tests.
Duration of symptoms: The duration of symptoms for scrub typhus can vary. Typically, symptoms resolve within seven to 14 days of treatment. However, some patients may experience prolonged symptoms or complications.
Does your uncle have any comorbid conditions like sugar, BP (blood pressure), thyroid, or any addiction? Please provide details.
I hope the above information helps you.
Thank you.
