Hello doctor,

I sustained a penile injury last night during sexual activity. I noticed a blood-filled bump the previous night, and it has ruptured and become flat now. I do not have any pain. I guess it to be a ruptured vein. Kindly help me to know if it requires medical care.

Classical penile fracture is when you hear a pop sound, and you immediately lose erection. It will be painful and require emergency surgery to repair the corpora. Yes, it could be the superficial dorsal vein. There are many veins on the penis, so sacrificing one vein should not be an issue. The vein, once injured, will get thrombosis and usually will be blocked, but blood will drain to other tributaries, and the erection will happen soon after healing. So do not worry about that. Since no surgical or medical intervention is needed, you need not visit an emergency room.

Thanks for the reply.

Does a permanent lump develop where the vein is ruptured? If yes, is there anything else like massage, compression, wrap, Ibuprofen, Acetaminophen, Aspirin, heat, or cold packs that I can do to help it heal better and avoid a permanent bump? I used a syringe to poke a small hole in the middle, and some blood came out. I used a new needle and carefully disinfected the area. Is this a good idea?

Once the vein gets thrombosed and blocked, it will not recanalize again. A thrombus usually forms inside the vein, leading to a non-functioning vein that feels like a soft cord. You need not worry about it. There is no need to poke the bruised area, as the blood inside will be absorbed with time, leading to complete resolution of the blood clot. In the long term, you can observe your erection.

If you find any deviation of the penis after healing, it might be due to the fibrosis of the tunica albuginea layer. It is called Peyronie's disease. For now, I suggest you avoid intentional erections for a week to help in the healing process. And also, avoid intercourse for six weeks. There is no need for a massage, as it will worsen the condition. At this time, ice packs are not required. No need for medications either.
