What could cause missed periods with negative pregnancy tests?

Hello doctor,

I am 33 years old and my weight is 152 pounds with height:5 feet 6 inches. I have missed my period for two months and I have checked pregnancy in between and got negative twice. I do not have any medical conditions till now. I am enclosing the results of TSH, CWC, and abdominal ultrasound. Kindly suggest.

I understand your concern.

I have gone through your results (attachment removed to protect patient's identity), they are normal except for the thickened endometrium and slight fluid in the POD (Pouch of Douglas). You are having anovulatory cycles (you are not ovulating), that is why your period has not come. The next thing to do in the process of finding the cause of this delayed period is not to do a progesterone challenge test. A medication would be prescribed for you to take for five days. You are supposed to see your period within seven days after completing the medication. If you see your period, it indicates that the problem is an anovulatory cause which commonly is caused by PCOS (polycystic ovarian syndrome). It could be caused by medication also, especially if you have taken any within the last four months or a functional ovarian cyst in the ovary. If you do not see your period, there would be a need to do a hysterosalpingography (HSG) to see if your uterus has closed up. Check the treatment plan for the prescription of medication.

Tablet Primolut N (Norethisterone) 10mg 12 hourly for five days.

Thanks for the reply.

The issue that I mentioned about endometrium thickness has been resolved before. I just mentioned it I had the same two years back that too after one miscarriage. Currently, my concern is regarding the missed period. I do not have any discomfort, pain, or difficulties. I have taken vitamin D supplements for the last two months and stopped now. Should I need to take a tablet of Primolut N 10mg twice a day?

Yes, there is a need as the longer the delay, the heavier will be you period when it comes. This might affect your daily activity and could tip you into anemia. Once you are sure you are not pregnant, it is advisable to induce a period after six weeks. This will help to prevent the above. I hope this information helps you.
