Hi doctor,
I am writing this for your advice and your view on the relationship between symptoms for underlying causes, and holistic evaluation. I have chest pain, tightness, shoulder pain and stiffness. I do not know if I have psoriasis or eczema in the left elbow mainly, with itching for 19 years. I have inflamed big toes for two days as in gout. I have a seborrheic cyst in the center of the chest. I have elevated cholesterol levels total 266 mg/dL, triglyceride 147 mg/dL, HDL 48 mg/dL, LDL 222 mg/dL, non-HDL 217 mg/dL, and hemoglobin 10.6 g/dL. My reports suggest grade 1 fatty liver, mildly enlarged, and endometriosis of the uterine wall with two small fibroids. I have also brain fog. Fasting sugar was 112 mg/dL and BP was 160/80 mmHg and 140/80 mmHg. I had abdominal fat and thyroxine sodium 75 mcg. I feel my chest pounding and ringing in my ears sometimes. I have had warts in my neck and face recently. I have sensitivity to sound light. I cannot let go of the fight or flight response, causing prolonged stress. I also have morning stiffness, and muscle pain in the fingers of hands and feet. Is this metabolic syndrome?

I have reviewed all the data you mentioned. Your case is complex with many possible differentials. Auto-immune diseases like systemic lupus should be excluded. I suggest complete auto-immune profile blood tests:
ANA (antinuclear antibody), double-stranded DNA (deoxyribonucleic acid), RF (rheumatoid factor), anti-CCP (anti-cyclic citrullinated peptide), anti-Jo, anti-LA, and anti-SCL70. Your chest tightness is worrisome for me, it requires more analysis as the coronary disease is serious. I am interested in following up on your case. Further discussion is required in order to analyze all data and bring all symptoms together. Please send me all the investigations you have done.
