Hello doctor,

I am a 36-year-old female. I usually have heavy periods, but this time I am experiencing a light flow, which has continued for four days, along with period pain. A recent scan confirmed the presence of fibroids.

I understand your concern.

In pelvic inflammatory disease (PID), the pain typically begins before the onset of menstruation and is relieved once menstruation starts. In adenomyosis, the pain starts with the onset of menstruation and continues with the same intensity throughout the period. In endometriosis, pain usually starts two days before menstruation, worsens with its onset, and intensifies with each passing day, reaching its peak on the last day. Pain associated with an intrauterine contraceptive device (IUCD) usually starts with the onset of menstruation and may persist for a few days. I recommend the following tests for further evaluation: a midstream urine analysis and a pelvic ultrasound.

For management, you may take the tablet Diane-35 (a combination of Ethinylestradiol and Cyproterone acetate, two milligrams) once daily for three months. Start on the second day of your menstrual cycle, continue for 21 days, followed by a seven-day pill-free interval. Additionally, take a tablet of folic acid, five milligrams, once daily for three months. Please consult your specialist doctor to discuss this treatment plan and take the medications only with their approval.
