Hello doctor,

I have been experiencing a skin issue for about two weeks. I have developed two itchy bumps on my stomach, which initially appeared shortly after a hiking trip. Given their location and proximity (about 2 centimeters apart), I initially suspected insect bites, but I am unsure now. After the first week, they appeared to be improving, with reduced swelling and itchiness. But they have recently become itchy, red, and swollen once again.

What are the possible causes and appropriate steps to address this skin concern? Please help.

It appears to be a hypersensitivity reaction to insect bites with a possible secondary bacterial infection. I suggest you do the following:

  1. Apply Fucibet (Fuscidic acid cream with Betamethasone) or FM derma (Fluticasone, Propionate, and Mupirocin) cream twice daily for a period of 7 days.
  2. For bathing, please use Cetrimide bar soap for the next 2 months.
  3. Ensure that you decontaminate your surroundings.
  4. When applying the cream, make sure to use clean hands or a cotton tip applicator.
  5. You can also apply a 5 percent Permethrin cream on day 1 and day 7, concentrating on the affected areas. Wash it off after a contact period of 8 to 12 hours.
  6. Maintain proper hygiene, as this plays a significant role in preventing a recurrence.
  7. Avoid scratching or rubbing the affected areas, and keep your nails trimmed.
  8. Consider taking Tablet Levocetirizine 5mg at night after eating for 7 days if itching persists.
  9. You can also take Tablet Ivermectin 12 mg once a week for 2 weeks.

Consult your specialist doctor, talk to them, and then take the medications as per their advice. In the future, take antiseptic measures and precautions while traveling. Stay aware and cautious, and try to minimize stress and maintain a relaxed lifestyle.

I recommend a follow-up after 2 weeks.
