What could be the reason for pain in the lower right part of the abdomen, occasional edema in the leg and constantly swollen glands in the neck?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am concerned about the low white blood cell count and neutrophil count, which has shown an increasing pattern in my bloodwork. This started two years ago and is getting worse. I get night sweats, low-grade fever and have serious digestive issues. My doctor said it is probably because I am overworked or stressed. But I am still worried. Please share your opinion on these reports. I am 5 feet and 3 inches, but weigh only 105 pounds, and am always tired. I have pain in the right lower part of my abdomen a few hours after eating, edema in the leg sometimes, and swollen glands in my neck all the time. I feel like something is not right. Please help.

Thank you.

Hi,

Thank you for stopping by!

I am here to provide expert opinions on your medical issues.

I have reviewed your reports (attachments removed to protect patient privacy) and it appears that you have dyslipidemia (an imbalance of lipids). Your low white cells and neutrophils are just a normal finding, do not worry about it. Your monocytes are a bit high but that could be because of a recent upper respiratory tract infection. Dyslipidemia, which is evident is of prime importance and it could be because of either Anorexia nervosa (an eating disorder in which people stress about their weight and what they eat) or any medications which you take long term. I suggest you do the following:

1. Eat a healthy low-fat diet with more fruits and vegetables.

2. Avoid being empty stomach for long hours.

3. Exercise daily for 30 minutes.

Please get your thyroid function and vitamin D levels checked.

You can always come back and reach me at icliniq.com.

Thank you.

Investigations to be done

I suggest you get your thyroid function and vitamin D levels checked.

Treatment plan

I suggest you follow a low-fat healthy diet with more fruits and vegetables. Avoid being empty stomach for long hours. Exercise daily for 30 minutes.

Regarding follow up

Review after getting the laboratory results.
