How can I take vitamin supplements with Accutane safely?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I will give you all my supplements and would like them placed in the best order for the best effect with the least side effects. I am also on Accutane, and I would like to know if I can take milk thistle for six months or if it should be taken for a shorter period of time. I would also like to know about the oil from the oregano pill.

  1. Should I take it for two weeks and then stop, or should I take it for one week and then stop?

  2. What is the best duration to take it?

I also want to know if any of the supplements interact with each other. My supplements are probiotics 60 billion for women, vitamin C, vitamin D, iron, vitamin B12, magnesium glycinate, omega-3, oil of oregano, and milk thistle.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

Since you are on Isotretinoin (Accutane), it is essential to be cautious with supplements, as some may interact with your liver or affect absorption. A balanced sequence would be to keep probiotics, vitamin D, omega-3, and magnesium glycinate as daily core supplements, as these generally support gut health, bone strength, mood, and skin repair without stressing the liver.

Vitamin C and B12 are also safe to use daily, but iron should only be taken if you have confirmed low levels, since excess iron can cause side effects and interact with other minerals. When taking iron, it should be on an empty stomach with vitamin C to boost absorption. It should not be taken simultaneously with magnesium or probiotics to avoid reduced absorption.

Milk thistle is often used as a liver support supplement, but since Accutane already stresses the liver, you should not use milk thistle continuously for 6 months. Instead, short courses of one to three months are considered safer while monitoring liver enzymes through blood tests.

Oregano oil has antimicrobial properties, but it is best used for a term, usually one to two weeks, because more prolonged use may disrupt gut flora and lead to stomach irritation. If you are already on probiotics, taking oregano oil for one week at a time is generally safer, followed by probiotics to restore balance.

Overall, there are no severe direct supplement conflicts in your list, but the key points are spacing iron away from magnesium and probiotics, avoiding long-term oregano oil, and not taking prolonged milk thistle without checking liver tests. Because Accutane can elevate liver enzymes and affect lipid levels, we should review any liver support supplements and monitor your bloodwork regularly.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
