Hi doctor,

I was recently diagnosed with major depressive disorder and general anxiety disorder by my psychiatrist. She prescribed me Yelate, 30 mg in the morning, 0.25 mg Alzam in the afternoon, and 0.5 mg of Alzam with 10 mg Ivedal in the evening. However, I have been having severe anxiety attacks and not sleeping for more than five hours. The anxiety attacks have been more severe and frequent than before I started the medication. Also, my sleeping patterns have degraded significantly. My concern is that the medication may be causing this. Please advise me on how I should proceed.

I understand your concern.

Yelate (Duloxetine) can cause such problems in a few cases, so that cannot be ruled out. Kindly discuss with your treating doctor regarding substituting Yelate with SSRI (selective serotonin reuptake inhibitor) like Sertraline. For sleep, you can discuss with your treating doctor regarding replacing Ivedal (Zolpidem) and Alzam (Alprazolam) with Clonazepam. Also, following sleep hygiene steps and breathing relaxation exercises can help with anxiety.
