Can scrotal infections cause severe pain?

Hello, doctor,

I need to know if I have Fournier gangrene. I have been experiencing left scrotal pain, especially in the lower left area and behind the scrotum, for the past few days. I have had a urinary tract infection (UTI) for the past five months and have been on antibiotics, but I have been unable to clear the infection. Additionally, my immune system is dysregulated and is currently being evaluated by an immunologist, but no conclusion has been reached yet. I also have esophageal candidiasis and other infections. Is there any air, swelling, or edema in the scrotum wall according to the ultrasound? I am worried that I may have developed Fournier gangrene, as many urologists are not aware and have not thoroughly examined it from that perspective.

I understand your concern. There are no features indicating Fournier gangrene in the attached reports (attachments removed to protect the patient's identity). The testes appear normal. There are a few cysts in the epididymis and a mild varicocele, but these are not acute conditions.

I have severe pain in the left side of the scrotum, even when I am sleeping, and it lasts the whole day. I am unable to sit or sleep due to the pain. I have been feeling nauseated as a result of the pain for a week. Could you kindly let me know what the next step should be for diagnosis or which hospital and doctor I can consult with?

I understand your concerns. It is possible that you have recently developed a scrotal infection, and it might have been mild at the time of the ultrasound scan. This could be the reason why the report does not provide much information about it. It is important to consult a surgeon or urologist for a clinical evaluation and appropriate treatment. Additionally, wearing loose inner clothes can help alleviate the pain in the scrotal area.
