Patient's Query
Hello doctor,
I am dealing with such severe constipation that it is affecting every part of my life, and I feel too embarrassed to even talk about it. I have not had a normal bowel movement in over two weeks, and when I do go, it is so painful that I fear it. The straining is so intense that I get hemorrhoids and sometimes bleed, and I feel like I am never empty afterwards. My abdomen is always bloated and swollen, making me look pregnant, and the cramping pain is becoming unbearable.
I have tried every over-the-counter laxative, stool softener, and fiber supplement, but nothing seems to work regularly. I am afraid to eat because it only makes the bloating and discomfort worse. This is affecting my work, my social life, and my relationship because I always feel uncomfortable and self-conscious.
What could be causing such severe constipation, and why are the usual treatments not working? Could this be a sign of something more serious, and what treatments are available to help me return to normal? How can I manage it?
Kindly help.
Hello,
Welcome to icliniq.com.
I understand your concern.
What you are going through is genuine, and it is not just uncomfortable; it is truly disabling. The fact that you have not had a normal bowel movement in over two weeks, even after trying many treatments, and that you are experiencing pain, a feeling of incomplete emptying, severe bloating, bleeding, and intense cramping, shows that this is not just ordinary constipation. It is likely something more serious that needs a full evaluation and targeted treatment.
There are several possible causes for this kind of chronic and treatment-resistant constipation. One major cause is slow-transit constipation, where the muscles in your large intestine do not move stool along properly. Another is pelvic floor dysfunction, where the muscles involved in passing stool do not coordinate properly. This can make it feel like you are always straining and never fully empty, even if stool is sitting in your rectum.
It is also important to check for anatomic problems such as rectoceles (a bulge of the rectal wall) or more serious conditions such as intestinal obstruction, colonic inertia, or even neurologic or endocrine disorders like hypothyroidism, diabetes, or multiple sclerosis, which can affect how your digestive system works.
When constipation is this severe, ongoing, and does not respond to common treatments, especially when it is paired with symptoms like bleeding, bloating, cramping, and poor quality of life, it is time to get a full medical workup.
I would suggest the following investigations :
Colonoscopy to rule out structural problems or cancer.
Anorectal manometry to test how your pelvic floor and rectum function.
Defecography (a special imaging test) to see how your body pushes out stool.
Transit studies measure how long it takes for stools to move through the colon.
Blood tests to rule out things like thyroid disease or abnormal calcium levels.
The emotional and social effects of this condition are just as important to address. You have already done the right things by trying lifestyle changes and over-the-counter products, but your symptoms suggest that you need more advanced care, not just fiber and laxatives.
Treatment depends on the cause, but options may include:
Prescription medicines like Lubiprostone, Linaclotide, or Prucalopride help stimulate bowel movements more effectively than regular laxatives.
Pelvic floor physical therapy using techniques like biofeedback can be used if the issue is poor muscle coordination.
You deserve relief and a return to normal life. This is not something you have to suffer with in silence. It is time to speak with a gastroenterologist who specializes in digestive motility disorders. With proper tests and the right treatment plan, you can feel better again.
I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.
