Factor Xa (activated form of the coagulation factor X) inhibitor inhibits platelet activation by selectively and reversibly blocking the active site of factor Xa without requiring a cofactor (coagulation factor) (e.g., antithrombin III) for activity. It also inhibits free and clot-bound factor Xa and prothrombinase activity; it has no direct effect on platelet aggregation but indirectly inhibits platelet aggregation induced by thrombin.
The blood coagulation cascade is dependent on the activation of factor X to factor Xa via the intrinsic and extrinsic pathways, which play a central role in the blood coagulation cascade. Premature discontinuation of any oral anticoagulant, including Apixaban (a highly selective direct factor Xa inhibitor), increases the risk of thrombotic events (thrombotic events refer to conditions in which blood clots form within blood vessels, obstructing normal blood flow); consider using another anticoagulant if anticoagulation with Apixaban is discontinued for a reason other than pathological bleeding or completion of a course of therapy.
An increased rate of stroke was observed following discontinuation of Apixaban in clinical trials in patients with nonvalvular atrial fibrillation (a type of atrial fibrillation that occurs in the absence of significant valvular heart disease). Increase in the risk of bleeding and can cause serious, potentially fatal bleeding.
Also, advise patients of the signs and symptoms of blood loss and to report them immediately or go to an emergency room. Discontinue the therapy in patients with active pathological hemorrhage.
Co-administration with other drugs that affect hemostasis (the body's natural process of controlling bleeding and maintaining blood within the blood vessels) increases the bleeding risk (e.g., Aspirin and other antiplatelet agents, other anticoagulants, Heparin, thrombolytic agents, SSRIs (Selective serotonin reuptake inhibitors), SNRIs (Serotonin-norepinephrine reuptake inhibitors), NSAIDs (Nonsteroidal anti-inflammatory drugs).
