This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I was suffering from anal pain. I am not sure if it is piles or anal fissures, but I have some pain outside my anus, and my stool was very hard, and I feel like I have cuts around my anus, which is very painful.

Kindly suggest what medicine I should take.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

To treat hard stools and cuts around the anus (anal fissures), the primary focus is on softening the stool and promoting healing of the tears. This involves a combination of dietary changes, hygiene practices, and potentially medications or, in some cases, surgery. You might probably have an anal fissure.

I suggest you make the following dietary and lifestyle changes:

  1. Increase fiber intake through fruits, vegetables, and fiber supplements to bulk up and soften stools.

  2. Drink plenty of water to help soften the stool and prevent constipation.

  3. Soaking in warm, shallow water for 10 to 20 minutes, especially after bowel movements, can soothe the area and relax the anal muscles.

  4. Use soft wipes or a gentle cleanser instead of toilet paper, and pat dry after cleaning.

  5. Avoid straining during bowel movements because it will worsen the fissure and prolong healing.

A few medications that can help you are as follows:

  1. Over-the-counter stool softener options like Docusate or Psyllium.

  2. Topical pain relievers like Lidocaine or other numbing creams can help relieve pain.

  3. Prescription medications or ointments like Nitroglycerin or Diltiazem to relax the anal sphincter muscles and promote healing.

  4. Botulinum toxin (Botox) injections to relax the anal sphincter muscle and allow the fissure to heal.

  5. Surgical options are usually reserved for chronic or severe cases.

  6. Consult a doctor if home care measures are not effective or if symptoms worsen. It is crucial to see a doctor for proper diagnosis and treatment.

  7. Healing from anal fissures can take time, so be patient.

I hope this answers your questions.

Best wishes.
