Hello doctor,

I have oily skin with dry patches, making it a combination skin. I am currently using a cream designed for this skin type. There are bumps all over my forehead, consisting of cysts. I have a habit of popping pimples, which releases a hard cyst-like substance. I have been dealing with cystic acne for five years, trying various products, herbal remedies, and homemade solutions, but nothing has helped. It started with a few pimples on my forehead and nose at the age of 12 or 13, but I did not pay much attention, thinking it was due to changes in my body. However, years later, it has worsened significantly.

There is not a patch of skin that has not been affected, and it is progressively getting worse. I did try Isotretinoin, but only for a month before discontinuing because it was no longer available to me. I have castor oil applied in the attached images, which makes my skin look oily, but you can see how horrible my acne is. I am really in need of advice or help.

It appears that you have oily skin and grade 3 to 4 acne. I suggest you take tablet Isotretinoin, which is the best treatment option for your condition. However, the duration of treatment will depend on your body weight and may require six to eight months. Additionally, I suggest you apply 3.5 % Benzoyl peroxide daily twice for local application and use a gentle cleanser twice daily. Before starting Isotretinoin, certain laboratory investigations are necessary, such as a liver function test and a urine pregnancy test if you are married. It is crucial to take Isotretinoin under the supervision of a nearby dermatologist. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

You should also follow certain dietary restrictions, including avoiding oily, spicy food, junk food, milk products, and non-vegetarian foods. Drinking plenty of water is important as it helps to hydrate your skin.
