Hello doctor,

I have tiny whiteheads on the top of my shaft and some in my pubic area that showed up only recently. These are not itchy or irritating; this is the first time such whiteheads have appeared. I had unprotected sex two weeks ago. I had a blood test done and received a negative result. I am not under any medications.

Kindly help.

I understand your concern and reviewed the image provided (attachment removed to protect the patient’s identity). It looks like a viral infection. If you had unprotected sex, it could be a sexually transmitted infection. I suggest you to follow up with the reports of the following investigations.

  1. TPHA (Treponema pallidum hemagglutination) test.
  2. HPV (human papillomavirus) DNA PCR (polymerase chain reaction) test.
  3. HSV (herpes simplex virus) 1 and 2, serum IgM (immunoglobulin M), and serum IgG (immunoglobulin G) tests.
  4. Chlamydia trachomatis IgG antibody test.
  5. HIV (human immunodeficiency virus) 1 and 2 antibodies test.
  6. HbsAg (hepatitis B surface antigen) test.
  7. Urine complete analysis.

In addition, I suggest you to follow a few instructions mentioned below.

  1. Such lesions can also be due to a simple bacterial infection. In case of a bacterial infection, you can use ointment T Bact (Mupirocin) for local application and take the tablet Doxycycline 100 mg twice daily for five days. Kindly consult a specialist, talk to them and take medications with their consent.
  2. Wash your genitalia twice a day (morning and night) with normal water (avoid cold or hot water).
  3. Avoid using soaps with heavy pH.

Thank you for the reply.

Can I apply Hydrocortisone cream? I think this may be a viral infection; I will leave it for seven days and see if this clears up before I go to a sex clinic.

I understand your concern. But since we suspect viral infection should not use topical steroid ointment (Hydrocortisone). It will flare up the situation.

It is better to go for T- Bact (Mupirocin) ointment. It will help prevent secondary infection and is safe to use on the genitals.
