Why do I have nausea and diarrhea after using the AF 50 Kit?

Hello doctor,

I had vaginal itching and irritation, so the doctor prescribed me an AF 50 Kit, Alerdain M Tablets, and Candiderma Cream. However, the AF 50 Kit is causing me nausea and diarrhea. I am worried. What steps should I take to manage this?

The AF 50 kit has three strong drugs together: Fluconazole (antifungal), Azithromycin, and Ornidazole (Antiprotozoal). This combination is often given when it is unclear whether the infection is fungal, bacterial, or mixed. But yes, it commonly causes stomach upset, nausea, and even loose stools. That is not unusual. One dose may be okay, but if you already feel unwell after taking it, do not keep repeating it.

Vaginal itching and irritation for two days may be just a simple yeast infection, which does not always need such a heavy triple-drug combination. A topical antifungal cream and sometimes a single oral antifungal are enough. The possible cause would be a vaginal yeast infection (Candida), and gastric side effects due to the AF 50 Kit.

However, the following investigations are advisable:

  • Vaginal swab and culture (to confirm yeast or bacterial infection).

  • Urine routine test if burning during urination is present.

I would suggest the following measures:

  • If nausea and diarrhea are severe, stop further doses of the AF 50 kit and inform the prescribing doctor.

  • Continue the prescribed external cream only for now. But remember, this cream contains steroid + antibiotic + antifungal. We usually prefer plain antifungal cream (like Clotrimazole cream) without a steroid for safety.

  • Maintain hygiene: use cotton underwear, avoid scented washes, and keep the area dry.

  • Oral antihistamines such as Alerdain M (Montelukast and Bilastine) tablets can reduce itching, but the main cure comes from antifungal treatment.

If symptoms persist for more than three to four days, you should get a local swab test and take a more specific antifungal course instead of broad triple-drug therapy.

Please update me if itching continues after three to four days of local treatment. If loose stools persist, take plenty of fluids. If severe, stop the kit immediately. Follow up with me or any gynecologist for a proper swab-based diagnosis before starting stronger medications.

Preventive measures include:

  • Change pads and undergarments frequently.

  • Do not sit in damp clothes.

  • Avoid unnecessary antibiotics as they can trigger yeast overgrowth.

  • Keep sugar intake minimal since yeast thrives on sugar.
