Hello doctor,
I went on a road trip for two weeks. Upon returning, I noticed a rash on both sides of my feet. I observed these bumps three days after the road trip. The bumps are not painful and do not itch. I initially thought they might be scabies, so I treated them with Permethrin. However, there has been no change even after applying it twice. Currently, I am taking Valacyclovir, Gabapentin, Hydroxyzine, Famotidine, iron supplement, and thyroid medications.

I have reviewed the attached pictures (attachment removed to protect the patient's identity). It seems there are multiple urticarial erythematous papules on the exposed area. The possible differentials include papular urticaria, allergic contact dermatitis, and urticarial vasculitis. Permethrin might not be effective in this case, as scabies are usually itchy and tend to affect the upper limbs first. You would benefit from applying topical steroids, preferably Mometasone (Mometasone furoate) ointment, once daily over the lesions for seven to ten days to achieve complete clearance.

Yes, that would be beneficial. You can also take antihistamines alongside it to help alleviate the redness. It is advisable to avoid visiting gardens or parks until this issue is resolved.
