Hi doctor,

I have had more than usual fatigue, shortness of breath, frequent nosebleeds, and chest pain occasionally. Also, I keep getting back-to-back urinary tract infections, for which I have been on antibiotics frequently. I was in the emergency room about one and a half weeks ago. My doctor said I was fine, but my blood reports were slightly off. My test report shows abnormal levels of RDW-CV - 11.4 %, platelets - 86 per microliters, lymphocytes - 197/microliter of blood, CK 501 U/L, and osmolality - 264 mOsmol/kg.

Kindly suggest what needs to be done.

Your symptoms could be related to prolonged infections and the use of antibiotics. UTI (urinary tract infections) is cross-infection; hence, your partner must be checked for it as well.

1) Platelets are usually reduced in infections, but 86000 per microliter is only a grade 1 decrease and is insignificant, and your RDW (red cell distribution width) value is unremarkable.

2) Your lymphocytes are below 1.97 x 10e3/mcL which is not in the normal range, indicating the presence of infection in your urine.

3) Your CK (creatinine kinase) is clinically high. CK is a muscle breakdown product that is increased in prolonged fasting, TPN (total parenteral nutrition), dieting, heart or skeletal muscle damage, and prolonged ailments.

4) Bleeding symptoms might be related to vitamin C, vitamin K (due to prolonged use of antibiotics), and vitamin B12 deficiency. I suggest you increase your vitamin B12, folic acid, pyridoxine, vitamin D, zinc, vitamin C, iron, and coenzyme Q10 levels.

5) For osmolality, take plenty of water with locally available oral rehydrate salts.
