Hi doctor,

I have not been feeling well, so I had a urinalysis done. I did start my period later that day. Are these results inclusive of starting your period? Can you help with the results of my urinalysis?

The urine analysis report (attachment removed to protect patient identity) shows urinary tract infection. Did you have a fever, or urinary symptoms like night time frequency, sense of incomplete voiding, an urgency to pass urine? When urinating, does it pass freely or feeling obstructed or need to strain? It is mostly simple cystitis, that is, lower urinary tract infection without fever, which is quite common in young sexually active females. I suggest you try taking the tablet Tamsulosin 0.2 mg at bedtime, and tablet Nitrofurantoin 100 mg at bedtime for five days.I suggest you to follow preventive measures for UTI which are, do not hold urine for long, wash the genital area before sexual intercourse, use good lubrication, like saliva, and always urinate after having sex, as it washes out bacteria introduced due to the short female urethra and not giving time for it to multiply.

How do you know the difference in a kidney infection versus a bladder infection?

Kidney infection has associated flank pain and fever, which is high grade with chills and rigors. For your symptoms and the urine analysis, a simple cystitis is more likely. No further testing is required at present. Kidney infection in investigation reflects as raised TLC (total leukocyte count) counts in CBC (complete blood count), if the patient is ill, a CT scan may be done, which can show changes of pyelonephritis. You may take the suggested medicines and if no improvement in three days, I will suggest further investigation, but I am hopeful that you will have improvement.
