Hello doctor,

I have stress incontinence for more than ten years. I leak when I sneeze, laugh, or jump. Also, I have a protruding abdomen.

Thank you for consulting us. I understand your concern. Stress incontinence is mainly caused by weakening pelvic floor muscles leading to urethral hypermobility or a weak urethral sphincter. It sometimes occurs in combination with an overactive bladder. Urodynamic studies are essential to role out an overactive bladder. In addition, other investigations like urine analysis, urine culture, KUB (kidney, ureter, bladder) X-ray, ultrasound scan, and serum creatinine are important. If stress incontinence is diagnosed, treatment can be from pelvic floor exercises to intraurethral fillers to slings depending on the degree of incontinence. From your short presentation, it seems to be mild; therefore, it is better to go for conservative treatment like pelvic floor exercises.

It hinders my day-to-day activities as I occasionally get wheezing and tend to sneeze more. I will get some tests done. Can you advise me on what tests need to be done on priority?
