Hello doctor,

I am 29 years old and six months postpartum. I have had ulcerative colitis for five years, managed with Mesalamine. Lately, I have been seeing blood and mucus again, with urgency and joint pain.

  1. Could hormonal changes trigger a flare?
  2. I am still breastfeeding; can I stay on mesalamine, or do I need something stronger?
  3. Also, are there safe anti-inflammatory diets I can follow while nursing?

Are postpartum hormonal changes triggering your flare?

Yes, hormonal shifts can absolutely trigger a UC (ulcerative colitis) flare. After childbirth, estrogen and progesterone levels drop rapidly, which influences immune regulation and gut inflammation. Sleep deprivation, stress, and changes in the microbiome play a role.

Can you start on Mesalamine while breastfeeding?

Yes, it is safe during breastfeeding. Very little medicine passes through breast milk. They are the first-line treatment for UC and safe for both mother and child. If symptoms worsen, then a higher oral dose is needed, adding rectal Mesalamine.

Can you follow an anti-inflammatory diet?

Yes, with some precautions. Diet will not cure UC but can reduce inflammation and support healing. Low FODMAP (fermentable oligosaccharides, disaccharides, monosaccharides, and polyols) diets are usually safe.

Take laboratory tests for monitoring:

  1. Fecal calprotectin.

  2. CBC (complete blood count).

  3. CRP (c-reactive protein).

  4. Sigmoidoscopy.

  5. Colonoscopy.
