Hello doctor,

I am 40 years old and have been living with type 1 diabetes for many years, but recently managing my condition has become more difficult. Over the past year, I have gained 25 pounds, and it is been challenging both physically and emotionally. My cholesterol levels are high, and my kidneys are showing early signs of damage, which is deeply concerning.

My blood sugar levels are unstable, and I feel like I am losing control over my health. I am particularly worried because my father had complications from diabetes and ended up on dialysis, and I do not want to face a similar situation.

Is the recent weight gain contributing to my health issues, or is my diabetes progressing with time? I am unsure if my struggles are primarily weight-related or if there is more going on with my diabetes management.

Given my family history and current health challenges, I am seeking advice on how to better manage my diabetes, especially with kidney damage and high cholesterol in mind. What changes can I make to stabilize my blood sugar levels and protect my kidney function?
