I had a turbinoplasty with radiofrequency about 11 days ago. I have removed the splints on day 7. But it has been four days since I cannot feel pain, temperature, and the airflow in my noses. It first started with the left side, now on both sides. What can it be?

Can you please give us the surgery notes? Turbinoplasty should not cause this problem. Is there any crust formation inside? Did the ENT do an endoscopy after surgery and clear all debris and crusts (I mean after pack removal/splint removal)? What medicines are you taking?

Thank you doctor,

I revisited the surgeon, and he said there is crust inside, and it was turbinoplasty with radiofrequency + septoplasty. The doctor said he was very conservative. He did not remove it because he said it is adhered and could crust again. But despite the doctor's explanation, I would like to have a second opinion. I can only smell, but I put my head in the refrigerator and breath deeply. Although I feel the cold in my skin, I cannot feel the cold airflow going through my nose. I can feel only when it gets to the throat. The same with 3% saline, which use to hurt my nose, and now I can use it as much as I can, and I feel nothing, only when it hits the mouth.

Use the saline nasal douching three or four times daily vigorously. Then revisit your primary surgeon and ask him to suction everything nicely. After that, you will find a good improvement. Avoid breathing cold air as it will worsen the crusting. Use a steroid spray after the crusts are out. Drink plenty of water. The presence of crusts inside will cause the problems described by you.

I have not found anything like this in the lecture, even talking to friends who went through the same situation, and I would kindly like to know if you could provide me more information about this condition. Is it empty nose syndrome or atrophic rhinitis? How long should I expect improvements? Also, are there any chance of this condition be related to the AC air? Since I went home from the surgery, I was under 16ºC AC air.

Your operation was just 11 days ago. For the mucosa to become completely normal post-surgery, it will take at least three to four weeks. Until then, the mucosa (post-op status) will produce thick mucus and some blood, which will form crusts (like scabs on the skin), and they will be blocking the nose. So every four to five days, you will have to visit your doctor to clean the nasal cavities. Otherwise, you can douche the nostrils yourself with saline water multiple times daily to prevent crust formation. You must be in a room with a temperature of about 24-26 degrees, and 16 degrees is too low and will cause more crust formation than normal. Cold air is not good for sino-nasal mucosa. Naturally, you do not find all these details on the internet. You will have to read proper medical journals for that.
