I hear a whooshing sound in my left ear with earache for a week. Behind the ear, it is red and sore with spots.

I understand your concern. According to your statement, you are suffering from a whooshing sound in your left ear for a week with an earache and it is red with soreness behind the ear. Many people hear the whooshing sound or pulsatile tinnitus on one ear due to the turbulent flow of blood vessels in the neck or head. Such type of clinical features occurs due to conductive hearing loss. It occurs due to infection in the middle ear or due to accumulation of fluid or due to problems in the ossicles, accumulation of fatty plaque inside the carotid artery, blood vessel disorders like an aneurysm or arteriovenous malformation or glomus tumor behind the eardrum, or benign intracranial hypertension, high blood pressure, and anemia.
Treatment depends on the cause and you should undergo some investigations like magnetic resonance angiography, carotid artery ultrasonography, Weber test and Rinne test for conductive hearing loss, MRI (magnetic resonance imaging) of the brain, CSF (cerebrospinal fluid) study to detect the underlying cause for the present sufferings and then it needs to be treated accordingly.
