Hello doctor,

I am 68 years old, weigh 167 pounds, and am 5 feet 9 inches tall. I had bypass surgery seven years ago and used to walk for 45 minutes daily. I generally feel fit and healthy. However, a week ago, I noticed that my heart rhythm seemed irregular. After undergoing a Holter test, I found that my heart rate increased and decreased with a pause of 1.8 seconds. My minimum heart rate is 50 beats per minute, while my maximum is 144 BPM, and my average heart rate is 89 beats per minute. Is there anything serious to be concerned about?

Regarding my presenting complaint, I rarely get headaches and have no other symptoms. I am currently taking Razel Gold once a day. In the past, I have taken Concor 5 mg once a day and Razel Gold once a day. I underwent a treadmill test (TMT) three to four months ago, which had normal results. The doctor also performed an echocardiogram, and everything appeared normal.

It appears that you are experiencing episodes of supraventricular tachycardia, likely due to atrial fibrillation or flutter. Additionally, you have sinus pauses and sinus arrest, which suggest sinus node dysfunction. You will need treatment for the supraventricular tachycardia episodes, such as rate control medication and anticoagulation if it is atrial fibrillation. Currently, your sinus node dysfunction does not require any treatment. I recommend consulting a cardiologist to discuss starting treatment.
