Hi doctor,
The patient is an 80-year-old male theoretical physicist. Before a stroke, he was healthy. However, a stroke affected the left brain hemisphere a few months ago, and a thrombectomy was performed. Rehabilitation phase since the beginning of last month, and now at home. Physical condition satisfying, the right-hand writes bad, which is important for the patient. Cognitive function problems exist. Communication with the neurologist is complicated. The patient takes morning Aspirin, vitamin B3 complex, vitamin D3, and evening Atorvastatin. Finally, with visits with a physiotherapist, and an ergo therapist, the question with a native-speaking logopedist is getting cleared. Please give recommendations for post-insult rehabilitation. He is taking Atorvastatin, vitamin B12 complex, Piracetam, Phenylpiracetam, and Oxiracetam (advice needed, I persist Oxiracetam is significantly more effective).

The left hemisphere is an eloquent area. Post-stroke rehabilitation would include cognitive rehabilitation, which will focus on the cognitive domain involved, like, for instance, language. Nootropics such as Oxiracetam are good but will work best with aggressive cognitive rehabilitation. I suggest he undergo a cognitive assessment to check the domains involved in cognition. Aggressive rehabilitation for the first six months to one year after the stroke will benefit the patient best. Oxiracetam is good.
