I have had stomach upset for about one month. What to do?

Hi doctor,

I have been suffering from stomach upset for about one month. My stomach feels full and tight all day. My belly looks larger than usual. When I start to eat or drink stomach feels full, but I am still hungry. I have on-and-off mild stomach pain. I have throat irritation and a burning sensation on my tongue. I took capsule Omeprazole 20 mg once a day, but it did not work for me. Can you please help me with this issue?

The most common causes of stomach upset are gastritis, constipation, IBS (irritable bowel syndrome), infection, and accumulation of fluid in the abdominal cavity. Other uncommon causes are tumors of the GI (gastrointestinal) tract.

Out of these, gastritis should improve with PPIs (proton-pump inhibitors); constipation has episodic symptoms, and fluid accumulation (ascites) may have persistent symptoms if not treated.

To delineate the etiology, we should go for the detailed history and investigations. Can you please give me the following details:

  1. Do you consume alcohol? Kindly elaborate on the duration, quantity, and type of beverages.
  2. Do you smoke? Kindly elaborate.
  3. Do you feel pain in the abdomen?
  4. Is there any pain while swallowing food or liquids?
  5. Any related symptoms to the intake of food?
  6. Do you feel regurgitation of the food?
  7. Do you have constipation? Also, tell about the stool frequency and consistency.
  8. Do you feel episodic abdominal distension?
  9. Is there any relieving factor?
  10. Do you have any swelling in the feet?
  11. Have you had any investigation done for this ailment?

I would suggest the following measures:

    • Ultrasound abdomen.

    • Stool H. Pylori antigen.

    • Blood Investigations:

    1. Complete blood count.

    2. Liver biochemical tests.

    3. Kidney function tests.

    4. Thyroid function tests.

    5. Lipid profile.

    6. Fasting blood glucose.

    7. HbA1c (glycated hemoglobin test).

The following conditions need to be ruled out:

  • GERD.
  • Gastritis.
  • IBS.
  • Ascites.

I would suggest the following measures for treatment:

Tablet Dexlansoprazole 60 mg once a day on an empty stomach. Psyllium husk: two scoops in 200 ml of water or milk in the morning. Avoid junk foods. Avoid carbonated drinks, alcohol, or smoking, if any. Drink plenty of water. Regarding follow-up.

Please see below my comments: Do you consume alcohol? (Kindly elaborate on duration, quantity, type of beverages, and duration). Once a week, Beer one liter or three pegs. Do you smoke? (elaborate). No (I stopped smoking three years ago). Do you feel pain abdomen? No. Any pain while swallowing food or liquids? No. Any relation of symptoms to the intake of food? No. Do you feel regurgitation of the food? No. Do you have constipation? Also, tell about the stool frequency and consistency. No, daily morning and soft (normal). Do you feel abdominal distention is episodic? Yes. Is there any relieving factor? No. Any swelling in the feet? No. Have you had any investigation done for this ailment? I just did a blood test. Please see the attached report.
