Hello doctor,
I wanted to know if you see any possible indications of an early pregnancy on my ultrasound pictures. I had spotting to a very light flow for five days. My periods are normally light for 1 to 2 days, increasing to a heavy flow for two days, and tapering off for another two days. I normally have dark red blood with clots and painful cramps. This month when I had the spotting bit, it was light pink to light red, no cramps, and never became heavy. My cycle is normally 35 days. I am currently on Vascepa 4 g per day, Metformin 2000 mg per day, Fenofibrate 160 mg per day, and a prenatal vitamin.

I have gone through all the pictures of the ultrasound sent by you (attachment removed to protect patient identity).
There is endometrial thickening evident, but there is no apparent evidence of any early gestational sac in situ, which would indicate any pregnancy.
As you described, you had dark red blood with clots and painful cramps. But this month, you had light spotting, no cramps, or heavy bleeding.
So, if you suspect or expect pregnancy as of date, the best way to rule out or confirm pregnancy in such early cases (if it is so) is to get a serum beta hCG test done, which is a blood-based test. It has a sensitivity of detecting pregnancy after seven days post intercourse.
If the hCG levels are more than 5 mIU/mL, then you are pregnant for sure; else, you are not.
