Hello doctor,

My wife underwent IVF last month with a fertility expert. Post IVF procedure we had done hCG test three times as asked by our fertility expert. However, in the last few days, she experienced bleeding and spotting multiple times. Also, yesterday was her first sonography post six weeks, but she experienced bleeding. However, in the ultrasound, it was revealed that the baby's heart or fetal heart rate was 103 BPM and with no internal bleeding. Upon consultation visit to our fertility expert, she said we are not sure what is the cause of bleeding it can be implantation related or it can be even missed. But seeing the reports of ultrasound it seems everything is fine. Yesterday she was asked to change the dose which she is taking post every four days after IVF. Even today she experienced spotting. Please help us know if everything is going fine with her.

I have gone through your history and report and understand your concerns. As your ultrasound shows everything is fine, so it will be fine. Get your blood beta hCG (human chorionic gonadotropin) level monitoring. Are you bleeding and having pain? If not, no need to worry about it. Sometimes early conception cause these findings. Take folic acid tablets and progesterone to support pregnancy. Rest and repeat ultrasound after two weeks.

Hello doctor,

Post IVF when we had eight weeks of sonography done, the baby and sac were not there. Our doctor told us that its a case of missed abortion, she said we need to again attempt the IVF post 2 to 3 months later. She has frozen our two embryos. Will this approach work this time with those embryos frozen? As she said probable cause for failed pregnancy could be anything from blood supply issues, genetic issues in embryos, autoimmune, etc. I asked her to do a PGT test which she said it cannot be done on frozen embryos, is that the case?

Also, my wife had a polyp removal some two years back. The doctor says she did not find any polyps while scanning. She also said we will need to do a hysteroscopy of my wife. Also as a safety measure, she said that my wife needs to rest completely post-IVF. We are really worried about this failed issue. Please let us know if it will work. I am attaching the sonography report that was taken yesterday.

Here are the answers to your questions. Yes, she is right, early pregnancy losses are generally due to chromosomal abnormalities. Yes, PGT (preimplantation genetic testing) cannot be done on frozen embryos. Also, polyps are a recurrent condition but in your case, if they are not seeing it, it may not be there. She wants to do a hysteroscopy to rule out any abnormalities before going for the next IVF (in vitro fertilization) cycle. You should not worry about it, you can go for IVF cycles for four times for successful IVF pregnancy.

Hi Richa Ma'm I m Aniket Rathod. I had consulted you in past for my wife ,who underwent IVF process here in Mumbai. Post 1st IVF cycle she had miscarriage post 1.5 month in July 2023.However after this our Fertility doctor said we need to wait for 3 months time post which only we can do for next cycle However we on 18th Nov 2023 my went through again Embryo transfer.She was due to get tested on 30 Nov but result showed she is not pregnant. Last time at least pregnancy stayed for 1.5 Month but this time it didnt even stayed for that time. She was given Evaporin Injection over and above her progesterone's injection (every 4 days) Question : 1. What could have happened this time that even she didn't test positive even i her first Beta HCG test ? 2. Also i wanted to ask can too much bed rest could have caused this ? She was asked to be in house arrest. But my wife did not move out of the bed too often or do chores like cooking or any other labor work 3. I also wanted to know can this mean there is some fundamental problem in Embroyo in our case ? 4. Do we need to go through any kind of test to determine any cause that is going unnoticed ? 5. Lastly what are the success chances for IVF multiple cycles ? Regards Aniket Rathod

hi, thanks for your reply. Answers of all your questions is there is only 20-30% of chances of pregnancy. Maybe there is problem in embryo. No clear cause sometimes to rule out why pregnancy nit happened. too much bed rest not required. Embryo quality can be checked before implantation. hope found useful Regards
