Hello doctor,
I recently had my premolars on the right side of my face, number four upper and five lower, removed, and now I am regretting it after reading all the negatives on premolar extraction. What can I do? I am only 18 and cannot have dental implants. I have wisdom teeth coming in. Can I use these to fill in the gaps where my premolars were?

I do understand your concern.
See, premolar extraction is done only in case of crowding in orthodontic treatment. You must have read about facial deformation, but major deformation is caused by canines.
Lastly, you are in the growth phase so any deformation will be balanced by drift. I request you to do not worry, all space lost will be managed by jaw growth and the natural migration of teeth.

Thank you for the response.
I do not necessarily have any crowding on my lower jaw, but I do have an open bite and my lower teeth are slightly off-center, I mean the middle line between my two front teeth is not in line with my bottom teeth, so I do not understand why my bottom premolars were removed when there is no crowding?

Thank you for the clarification.
See as per the clinical picture (attachment hidden to protect the identity of the patient) there is no crowding. But, as you are saying, there is an open bite, so there may be chances of either mandibular growth excess or flaring. In that condition usually, the premolars from one side of the lower and upper one are removed to maintain symmetry. Now the alignment will be done to remove the asymmetry and any deviations along with you will be given Frankel 3 to modify the open bite. So please be assured of good results.
