Hi doctor,

I have a hard white layer on head and glans of the penis. Please help. This problem is 4 to 5 months old and I am not taking any treatment. I am 19 years old. The white layer is not going even after washing it.

I read and understand your concern.

There is a white coating on glans and little bit redness on the edge (attachment removed to protect patient identity). It is smegma. It is due to excessive shedding of normal skin cells daily leading to smegma, where we can see a white coating on the glans penis. It is harmless. You can try Cetaphil cleanser for regular wash instead of soap and apply Alokem gel (Aloe Vera Extract, Wheatgerm, Pongamia Pinnata Extract) on the glans after the wash in the morning and Tacroz 0.03 (Tacrolimus) ointment bedtime for 10 days. After that, continue Alokeen moisturizer twice daily and stop Tacroz ointment. Smegma is harmless and it is not contagious.
