Hello doctor,

I am 19 years old. I have been to several doctors, but no one could comment on the painful rash I have on my face, body, and genitals. I have only been given antibiotics to stop and prevent the infection, but that has not helped me. I have had this problem for the past two months.

I saw the picture you have attached (attachment removed to protect the patient's identity). I can see plenty of cysts, erythematous papules (red bumps), and some crusting (yellowish deposit). When I just look at this picture, I think it is severe cystic acne, but you have mentioned that you have similar painful lesions on the body and private area. So this is becoming a little tricky. I would also like to see the other side of your face, including your forehead and back, where you have these red bumps. Can you please add a few more pictures for clarity? Please answer the following questions. How did this start? Was it painful from the beginning? Where did this start initially, on the face or the body? Do you have pustules as well? Did you have a fever when this started? Did you apply any topical cream? Were you given antibiotics? Do you have lesions on your palms and soles? Do you get flushing or redness on your face? Other possibilities I am thinking of here are eosinophilic folliculitis, extensive impetigo, acne agminata and severe rosacea.
