This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I have recently discovered a skin ulcer on my stomach, and I do not know how to treat it. I want to speak to someone if possible.

Hi,

Welcome to icliniq.com.

I understand your concern.

My sincere wish and attempts would be directed to aid you in getting over this annoying issue. However, before actually suggesting a specific treatment protocol, I need your responses to certain queries regarding the development of this non-healing ulcer over your abdomen:

  1. Which years, respectively, were the caesarian sections done, the time gap between the second section and the beginning of and duration for which your weight loss physical activity protocol lasted, and the development of the ulcer over the lower abdomen.
  2. Has the physical activity protocol been abandoned or is it being continued? As it would need to be halted till healing is decent. Other anti-healing factors include stretching forces, sweat accumulation, caustic agents like Betadine, Dettol, Savlon, aromatic oils, lime, other home remedies, tinctures, etc., and antibiotic creams.
  3. Please provide details of all applications and their response to the worsening or temporary improvement of the ulcer. Any history of specific trauma sustained, e.g., wood injury during gardening, etc.?
  4. Any general medical condition and corresponding treatments being taken or not being taken, especially - obesity (please provide your Body Mass Index [BMI] or weight (kg) and height (cm); iron or vitamin B12 or folic acid deficiency with or without anemia, diabetes, general debility.
  5. How is blood pressure controlled? Fasting sugar and HBA1C levels?
  6. Was recent vaccination or local subcutaneous injection/jab taken for any reason? E.g., any insulin or pain-killer shots or subcutaneous shots of heparin, etc.
  7. Any other symptoms - pain, tenderness, difficulty sleeping, oozing, bleeding. How much and how often?

There are many possibilities, although a delayed post-caesarian surgery panniculitis in which the skin seems to be ulcerated deeper till the level of the subcutaneous fat. Till then, general measures to be followed:

  1. Stop all vigorous physical activity, especially if it stretches the ulcerated area.
  2. Stop the application of any caustic solutions, soaps, and creams, as mentioned.
  3. Keep it dressed after cleaning with normal saline solution, applying 1-2% Fusidic acid or mucopain cream or ointment and a sterile gauze and pad anchored by a criss-cross micropore tape.

I will be able to help you once you:

  1. Respond to queries.
  2. Provide images of the ulcer and the involved area from a different angle- long and close-up shots.

Wishing you the best and earliest of recoveries.

Regards.
