Hello doctor,

I have a lot of pain in my right shoulder. I have difficulty lifting anything overhead and while doing any physical activity that focuses on the shoulder area. My shoulder also makes a lot of clicking and cracking that is quite painful. I previously did a steroid shot in that area to help, but I am still struggling. I did a recent MRI to understand what is going on but have not yet seen a doctor interpret the results.

In MRI (magnetic resonance imaging), your head is a little rotated, which might be due to spasm or congenital (attachment removed to protect patient identity). There is a capsular hypertrophy due to inflammation and irritation. And there is sub-acromion and sub-deltoid bursitis due to friction, irritation, and inflammation. In my opinion, we can take another trial of steroids, anti-inflammatory drugs, and physiotherapy. If things not improved with conservative management for three to four months, then arthroscopic surgery to remove the bursa needs to be performed.

Can you please explain my MRI report? You have used terms I am not familiar with. What do you mean the head is a little rotated? Also, I have been taking anti inflammatory medication and doing physiotherapy for a year now, but the pain is still persistent.

Humerus head joint with scapula glenoid and made gleno humeral hood normally. In your finding humerus head is a little rotated than the normal position, which may present since birth or may be due to inflammation. Bursa means spongy, cushion-like structure present beneath the acromion process (part of scapula bone) and beneath deltoid to prevent friction. In your MRI finding, there is some inflammation present, so bursitis developed. The capsule is a structure present at the glenohumeral joint, making a closed cavity at the joint and providing stability. Your capsule is hypertrophied (thickened, swollen, redness) due to inflammation. In my opinion, you can go with another dose of steroid injection, anti-inflammatory drugs, and physiotherapy for six weeks. If conditions still not improved, then surgery arthroscopic bursitis release and joint shaving is needed.
