Hello doctor,

I have been diabetic since I was 30 years old and have had heart issues for 15 years. I underwent bypass surgery eight years ago. Currently, my LVEF is 25 percent. Now, if I walk 50 to 100 meters, I feel breathless, and after having a bowel movement or urination, I again experience breathlessness. Could this be related to kidney issues? Also, could angiography and angioplasty be done without any risks?

Recently, I was admitted due to fluid retention in my lungs. In the 15 days after discharge, I still experience breathlessness and burping, which lasts for 15 to 20 minutes. Please let me know if this could be due to a heart block or kidney issues.

I reviewed your ECG (electrocardiogram) and would like to explain that it indicates a left bundle branch block. Regarding the shortness of breath, it is related to heart failure. Renal function may also be compromised, as this is very common in heart failure. I would like to know your current therapy. Fluid in the lungs is another common feature of heart failure. Optimizing your therapy could help improve your situation.

As for angiography or angioplasty, it can be performed safely in this clinical situation, if indicated by the doctor. However, it is necessary to first evaluate your kidney function by performing renal function tests to ensure there is no risk from the IV (intravenous contrast) contrast.
