My age is 47 years. I do not have any kids. Until the past two years, my wife and I had good sexual life. For past years I got mild diabetes and autoimmune disease. I cannot perform well in bed due to a short time erection. Please advise.

I can understand your concern. There are multiple reasons for your issue. The most common reason here is blood sugar and autoimmune disease.

Stress and anxiety can also be the cause.

These are my suggestions.

Choose an appropriate place for sex with no disturbance
Make sure you and your partner are fully naked before sex.
The room should not be very cold as it will shrink the penis
Always have dim light, almost as if you can only see the shape of your partner.
Admire your partner during sex
Give your partner an orgasm (read about female orgasm) before asking for foreplay from a partner.
You should receive at least 15 minutes of foreplay from your partner. During foreplay, your partner should always hold your penis stroke like masturbation, but very slow strokes should be continued till you insert it into the vagina.
Most men do not prefer talking during sex because of concentration issues. Subconscious words like loving you and sexy can be there, but strictly no complicated words.
Always start with missionary position. Then, post-childbirth, try scissors or doggy position.
