Hello doctor,

Since COVID-19, I have had a decrease in libido, a complete lack of interest in sex. Arousal does not occur even in frank moments. Attraction either disappears completely or manifests itself in rare moments; intercourse is perceived as a duty, and I simply cease to be interested in sex, even though I love my boyfriend; if sex does happen, the body does not react to the process.

Arousal disappears, lubrication is not released, and there are no emotions either; as a result, there is no orgasm. The process leaves one feeling disappointed or annoyed. I do not know what to do anymore, I have been to many therapists, but no one can find the problem.

Low libido has reasons such as:

  1. Infections.
  2. Diabetes and hypertension.
  3. Hormonal disturbances such as low testosterone, low levels of thyroid, and high levels of prolactin.
  4. Obesity.
  5. High cholesterol.
  6. Chronic disease or on any chronic medication.
  7. Anxiety.

You need to do USG (ultrasonography) pelvis to rule out pelvis infection, free T3 (triiodothyronine), T4 (thyroxine), TSH (thyroid stimulating hormone), lipid profile to know cholesterol level, blood pressure, and RBS (random blood sugar).

Kindly share more details regarding the above reason, so we can work on the cause and treatment. You can use the tablet Mildfil (Tadalafil) 5 mg twice a day as a temporary solution.

Hello doctor,

Thank you for your response.

Over the past four years, I have undergone several medical tests, including complete urine analysis, thyroid function tests, and hormone analysis (including testosterone, progesterone, and estrogen). Additionally, I have consulted neurologists and gynecologists, undergoing an EEG for neurological assessment as well as abdominal and transvaginal ultrasounds, all of which showed normal results.

I attribute the sexual pleasure issue to anxiety experienced during the COVID-19 pandemic and a severe earthquake in my area, both of which deeply affected me and were untreated at the time. Currently, I have changed therapists multiple times without finding a suitable match, and I have not engaged in consistent therapy due to what I perceive as a lack of major anxiety or panic attacks at present.

To gain clarity on whether this issue is organic, physical, or psychological, I am considering repeating the aforementioned tests. Regarding the medication you mentioned, Mildfil 5 mg, I am curious about its effects on females.
