Hello doctor,

I have hypothyroidism and started compounded Semaglutide (Ozempic) for weight loss, but ever since I began taking it, I feel super exhausted, my hair is falling out more, and my thyroid levels are off. I have lost weight fast, but I am worried if this is messing with my thyroid meds. Could Semaglutide be affecting my thyroid? Should I adjust my dose?

Semaglutide itself does not directly interfere with thyroid hormone levels, but rapid weight loss and changes in metabolism can affect how your body absorbs and utilizes thyroid medication, potentially leading to hormonal imbalances. Losing weight reduces the body's requirement for thyroid hormones, meaning that as you continue to lose weight, you may need a lower dosage. If you are still taking the same dose as before, you might now be slightly overmedicated, which could result in symptoms such as hair loss, fatigue, and exhaustion. It is important to check your thyroid hormone levels, including thyroid-stimulating hormone (TSH), free triiodothyronine (free T3), and free thyroxine (free T4). If these levels are significantly off, your medication dosage may need to be adjusted.

Semaglutide slows gastric emptying, which can interfere with the absorption of Levothyroxine if the medications are taken too close together. To ensure proper absorption and effectiveness, Levothyroxine should be taken at least four hours apart from Semaglutide. Additionally, rapid weight loss or undereating can put stress on the body, increasing levels of reverse triiodothyronine (reverse T3), which can block the active thyroid hormone (T3) from functioning properly. If you are losing weight too quickly, consider adjusting your Semaglutide dose or slightly increasing your calorie intake to prevent thyroid disruption. A healthy rate of weight loss is typically 0.5% to 1% of body weight per week; losing more than this may indicate excessive weight loss. Regularly monitoring your thyroid hormone levels and consulting your healthcare providers will help ensure that both your Semaglutide and thyroid medication dosages remain appropriate.
