Patient's Query
Hi doctor,
I have been diagnosed with scrub typhus, and I now understand that I have liver failure as well.
I would like to know what warning signs might indicate that my condition is becoming critical and when I should seek urgent medical care.
Please help.
Thank you.
Hi,
Welcome to icliniq.com.
I have read your query and can understand your concern.
Scrub typhus is an inflammatory disease caused by Orientia tsutsugamushi, transmitted through the bite of a larval trombiculid mite. The bite site often develops into a scab-like wound called an eschar.
Initially, scrub typhus presents with fever, chills, and rashes. If treatment is delayed beyond a week, it can progressively involve multiple organs, potentially leading to multi-organ failure. This progression can become a medical emergency and, if left untreated, may result in death.
With timely treatment, fever typically resolves within 7 to 15 days, and the body begins to recover and reverse the damage caused by the infection. The condition can be completely reversed and normalized by adopting a proper lifestyle, following a healthy diet, and adhering to treatment.
Treatment
Doxycycline capsules are the first-line treatment.
If Doxycycline is not tolerated, Azithromycin tablets may be used.
Supportive treatment and measures are also essential.
Prognosis: The liver, a highly regenerative organ, can completely heal and return to normal without leaving any signs of damage. Full recovery usually takes three to six months. The eschar at the bite site will also resolve.
Regular monitoring is necessary:
Complete blood count (CBC).
Liver function test (LFT).
Kidney function test (KFT).
Initially, these tests should be done every 3 months, then every 6 months, and finally once a year for two years.
Warning signs to monitor for complications: Scrub typhus can involve other organs, and the following symptoms should prompt immediate medical attention:
Brain involvement: Headache, seizures, or confusion.
Lungs: Breathing difficulty, reduced oxygen saturation, or severe cough.
Kidneys: Elevated creatinine levels or reduced urine output.
Gastrointestinal system: Severe diarrhea or persistent vomiting.
If proper treatment is received, a complete cure is possible.
I hope I have answered your query.
Let me know if I can assist you further.
Thank you.
