Patient's Query
Hi doctor,
My uncle (who is 55 years old) has scrub typhus and liver cirrhosis, complicating things. He has infected eschar and bleeding. He has increased ammonia levels, ascites, fever, and spontaneous peritonitis. How do we manage this? Should we do paracentesis?
Please help me.
Thanks.
Hi,
Welcome to icliniq.com.
I can understand your concern.
As per your query, your uncle has the following.
Infected eschar and variceal bleeding: The infection may have exacerbated the bleeding.
Elevated ammonia: Likely contributing to confusion, as ammonia toxicity can cause hepatic encephalopathy.
Ascites and fever: Suggestive of spontaneous bacterial peritonitis (SBP), which requires immediate attention.
Given the complexity of your uncle's condition, it is highly recommended to admit him to a tertiary care hospital with:
Multidisciplinary care team - Access to hepatologists, infectious disease specialists, and other experts.
Advanced diagnostic facilities - Availability of state-of-the-art diagnostic equipment and techniques.
Intensive care unit (ICU) - Close monitoring and support in an ICU setting, if necessary.
Feel free to follow up with any further questions or concerns.
Thank you.
