How to manage scrub typhus in a child?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My 5-year-old son recently attended a camp in a rural area and now has a rash and high fever. One of the parents mentioned scrub typhus, which I am unfamiliar with, and I am worried. He has also been complaining of belly pain and seems more tired than usual.

  1. What tests should we request to confirm if it is scrub typhus?

  2. Do we need to start antibiotics right away?

  3. What precautions should we take at home to prevent others from getting sick?

  4. Is it transmitted by insects or animals?

  5. And if he returns to camp, what steps can we take to keep him safe?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

Scrub typhus is a rickettsial disease caused by a type of bacteria transmitted through tick bites. Symptoms include fever, general fatigue, and a rash that often involves the palms and soles. A key sign is the formation of an Eschar at the site of the tick bite, typically found in the armpits or legs. It is a curable disease once diagnosed and treated.

Regarding your son's situation, I recommend seeing your pediatrician, who can begin investigating for scrub typhus. However, fever and rash in children are often caused by viral diseases, which usually resolve on their own. Based on the details you have provided, I cannot definitively diagnose scrub typhus.

To prevent scrub typhus, it is important to protect against tick bites. The disease spreads through ticks, not by contact or fomites, so you should avoid ticks in your home.

Possible causes of your son’s symptoms include viral infections, allergies, or various bacterial infections.

For testing, the screening test for scrub typhus is the Weil-Felix test, and confirmation is done via immunoassay or PCR to isolate the organism.

For treatment, fever can be managed with Paracetamol (10mg/kg every six hours) and tepid sponging. You should consult your doctor immediately if his fever persists, the rash worsens, he experiences increasing abdominal pain, lethargy, or any other serious symptoms.

I hope this helps answer your query.

Please let me know if I can assist you further.

Thank you.
